OF&A was contacted by a client who had been receiving threatening 'poison pen' letters via his home fax number. The client has also received a number of sinister phone calls, from someone claiming to be a member of a welfare organisation.

When OF&A investigated the matter with the welfare organisation's director, they discovered that the organisation was clearly not involved.

The investigation then moved to the client's employees, who were all interrogated. Certain suspects were identified, due to the suspicious manner in which they behaved and answered questions.

The suspects participated in polygraph examinations. In order to comply with union regulations, the shop stewards were also included in the examinations. One employee failed the examination and, after interrogation, confessed that he and a co-worker had been behind the whole incident. The information that he supplied led to the arrest of the main culprit.

It was ascertained that the two culprits were very unhappy with certain recent job promotions, the main culprit believing that he should have been promoted, but wasn't, as a result of prejudice towards him. Their shifts had also been changed suddenly, much to their dissatisfaction.

A disciplinary hearing was held, led by a neutral and independent lawyer from a well known law firm. Both culprits were dismissed, for committing acts sabotage and intimidation.
